Obtaining Disciplinary Certificates  (For licensure in other jurisdictions) 

All requests for a disciplinary certificate must be in writing.  The written request should include your name, current address, current telephone number, Missouri bar number, social security number (at least the last four digits), and the forwarding information for the certificate.  If you have a deadline that needs to be met, please state that in the first paragraph of the letter. There is a $5 charge for this certificate. You should send your request and a check to OCDC, 3335 American Avenue, Jefferson City, MO 65109-1079.
